PurposeTo assist service users with settling back into their home environment from discharge or by way of providing emergency response to care and support needs, NDHC provides a practical and emotional response via our Rapid Responder Team. NDHC recognises the importance of ensuring that all service user calls are covered, even when staff emergencies arise. The role of the Rapid Responder is at the forefront of these activities.Principal Duties and ResponsibilitiesThe Rapid Responder will, in line with their contracted hours of work: Respond to emergency care and support needs e.g., increase of home care and support packages, hospital discharges, emergency referrals, where home support workers are unavailable (sickness, annual leave, training, scarcity etc.).Provide a responsive flexible service to service users, supporting them in building their confidence and independence levels.Support in completing risk assessments, care planning, spot checks etc. as required.Supporting all home support worker duties until a regular home support worker has been identified.Travel as required to deliver person centred care and support.If required, assist the Community Management Team e.g. supervisors with the provision of information for the carrying out, organising and attendance of service user reviews, updating of needs assessment, care and support plans and all other risk assessment documents.Secure the safe handover of service users to their permanent home support workers.Work in partnership with the Community Management Team to ensure the safety of our service users at home.Undertake health and safety duties equal with the post and/or as detailed by the safety, health and welfare at work officer in line with company policy.Maintain a leadership stance at all times, ensuring that you lead by example, and upkeep safe, high standards in line with company policies and procedures.
